Ontario Cup Applications 

Team and Club Details:

• Official Team Name must be written exactly the same as what is writte on your EMSA Approved Roster. This enures that we can properly identify the team for approval.
• Team Name for Publication can be written however you would like to identify for the competition.
• Ensure you have entered the correct Competition 
  • The Competition should match the Divison the team plays in for league play

Team Official Requirements: 

• All team officials (Head Coach, Assistant Coach(es), Team Manager(s) are required to complete the Safe Sport Roster: 
  •  Respect in Sport for Activity Leaders
  • Making Headway in Sport
  • Making Ethical Decisions 
  • NCCP Emergency Action Planning
  • NCCP Understanding the Rule of Two 
• Each team entered in the competition must have a registered team Head Coach who has completed:
  •  Grassroots Diploma (and/or higher licensing)
  • The Safe Sport Roster as listed above 
• Assitant Coaches registered to the team must have completed:
  • Soccer For Life (and/or higher licensing) 
  • The Safe Sport Roster as listed above
