Trial Permit

This form is used when a player is not registered with any club.

A player may have two trials in any one season but each must be for a different club. 

 

Temporary Registration Form

 

This form is used when borrowing a player that is registered with another club.

 

De-Registration Form

This form is used when a player will no longer play in the current season.

If the player is going to play with another club the

Your Club Player registration forms must meet the minimum requirements for player registration as below or you can register your players to your club with the forms below.

Players and coaches are then assigned to teams and registered on the EMSA registration forms at the top of this page.

MINIMUM REQUIREMENTS FOR PLAYER REGISTRATION

Clubs may register players on their own forms as long as the club player registration forms

require all of the same information contained in the OSA player registration forms.

INTERNATIONAL TRANSFERS

 

*New ITC procedures released March 2, 2010*

(these new rules are for players 12-18 years)

 

EMSA requires the application number and proof of payment to the OSA before ITC's can be approved. This process can take as much as six weeks, so immediate notification is required.


 

INTER-PROVINCIAL TRANSFER REQUEST FORM  - there is no cost for this form but players who last played in another Canadian province must first get clearance to play in Ontario. Usual time for clearance is within the week of application.



TOURNAMENTS AND TRAVEL

teams participating in 'out of province tournaments/games' must provide proof of insurance to the EMSA office before your travel permit can be signed and stamped
