BELOW IS THE STEP BY STEP PROCESS
FOR YOUR COACHES OR MANAGERS TO USE WHEN APPLYING FOR THE
2010 ONTARIO CUP
1) The Ontario Cup Application form is on line.
This form is to be filled out and submitted on line. Please ensure you have completed it and have provided your club's email address.
2) Once the OSA receives your submission, they will email your club and request approval.
3) The club approves your application entry and notifies the District by email.
4) The District then uses the Switchboard and approves the application on - line.
5) You have three payment options,
Credit Card done on line at the time the application is submitted.
Payment by cheque directly to the OSA. This payment must be received by March 31, 2010.
Payment by cheque to the District by March 31, 2010.
There are no exceptions to this deadline. Failure to complete these tasks will result in your application being denied.
- please note that if you are paying by cheque EMSA REQUIRES that the cheque be submitted to the EMSA OFFICE (payable to the OSA) |
